So, open to U.S. residents only! Huh! The Dead are okay with having a Canadian run their archival releases but apparently not okay with them getting a chance at scoring some sneakers (no matter how gaudy they look . . let’s be honest). No harm: saved me some change, and no threat of any viruses crossing the 49th parallel. Cheers!

Just sat in line for over an hour to fill out the form only to see that the 'SUBMIT' button is not doing *anything*.
Attempted from both Firefox and Chrome.
Unsure if it's just not recognizing or giving any recognition of submission.
